COLLABORATION WITH PROJECT ECHO
(Extension for Community Healthcare Outcomes)

Project ECHO is a revolutionary guided-practice model that seeks to reach under-served groups who have limited access to specialist care.
Using case-based tele-mentoring, the ECHO model aims to amplify the capacity of the providers to deliver best practices in care and monitor outcomes.
